Boating Course Student Enrollment

As with any educational institution, there are forms to fill in and information to be gathered. Before we proceed, we want you to understand why we are asking for this information.

  • Name and Address - Required to ship the course materials.
  • E-mail address - Providing your e-mail address is optional. If provided, we will send you a message confirming your purchase and your Student Pin. It will also simplify communications with those who will provide you with state specific information and schedule examinations.
  • Phone number - Required so that we have a means of contacting you.
  • Date & State of Birth - If you should lose or forget your PIN we use this info to find it for you.
  • Power Boat Size - Optional info which we hope will help us determine what type of boating our students are involved in.
  • Sail Boat Size - same as above.
  • Other Boat Size - same as above.
  • Age Group - Optional info which we hope will tell us the age makeup of our student body.
  • Credit Card Number & Expiration Date - Required to purchase the course.
